According to a study in the Journal of Clinical Medicine, for patients with chronic spontaneous urticaria (CSU) undergoing omalizumab treatment, eosinophil/neutrophil/platelet/basophil-to-lymphocyte (ELR, NLR, PLR, BLR) ratios &ldquo;do not change significantly during 6 months of omalizumab treatment and do not appear to be useful in predicting its efficacy.&rdquo;Investigators conducted a retrospective analysis on patients with CSU who received subcutaneous omalizumab 300 mg every 4 weeks over a 6-month period. The study involved 52 patients, with assessments made at the beginning of treatment (V0) and after 3 (V3) and 6 months (V6). The study measured ELR, NLR, PLR, BLR, Dermatology Life Quality Index (DLQI), Urticaria Activity Score over 7 days, C-reactive protein (CRP), antithyroid peroxidase (anti-TPO) antibodies, and total IgE (tIgE) levels.Out of the 52 patients, 21 were responders to omalizumab, 24 were partial responders, and 6 were nonresponders. The patient cohort was divided into 18 patients with type I autoallergic CSU (CSUaiTI) and 34 patients with autoimmunity type IIb CSU with mast cell-directed activating autoantibodies (CSUaiTIIb).The study found that ELR, NLR, PLR, and BLR did not show significant changes throughout the 6-month treatment course.The investigators also noted that initial levels of ELR and BLR were significantly correlated with the initial tIgE level and the anti-TPO/IgE ratio. Initial ELR, NLR, and BLR values were significantly correlated with initial CRP levels.Additionally, patients with CSUaiTI had higher eosinophil and basophil counts, higher BLR, and elevated tIgE compared with type IIb patients. Conversely, anti-TPO and the anti-TPO/IgE ratio were lower in patients with CSUaiTI.The investigators noted several limitations to their study, including the relatively small sample sizes. Additionally, due to the retrospective nature of the study, patients were categorized into efficacy groups only after completing the study, and there was no randomization.They concluded their study, stating, &ldquo;The NLR ratio may be considered as a marker of inflammation in CSU, as initial values of NLR were positively correlated with initial CRP.
